Limited Xylitol Study Does Not Establish Causation

A Conference Presentation on Unpublished Science Should Not Outweigh Decades of Scientific Evidence

Scientific research is important, but not every study can be used to draw broad conclusions or create unnecessary concern among consumers. A new observational study presented at the European Society of Cardiology Congress suggests an association between xylitol and cardiovascular events, but the study has important limitations that must be considered before its findings are interpreted as meaningful for the general public.

The study is observational, has not been peer-reviewed or published in a scientific journal, and as its own authors acknowledge, it cannot establish that xylitol causes cardiovascular events. At most, the findings point to an association that requires further scrutiny.

Xylitol is recognized or permitted for use by leading health and regulatory authorities, including the U.S. Food and Drug Administration, the Joint FAO/WHO Expert Committee on Food Additives, the European Union, and Health Canada.

“Consumers deserve better than a rush to conclusions based on preliminary science. An ‎unpublished, non-peer-reviewed conference presentation is not a verdict, and this study’s own ‎authors admit this observational study cannot show xylitol causes cardiovascular events.  As ‎people who choose low- and no-calorie sweeteners are often already managing their weight or ‎blood sugar, conditions with well-established ties to heart disease—pinning the risk on xylitol is ‎backwards. More than 50 years of rigorous, gold-standard research back xylitol as a safe, ‎effective way to cut sugar and calories, and one presentation doesn’t change that.‎”

Study Design Limitations

The study’s observational design prevents researchers from determining cause and effect. This is particularly important because people who choose low- and no-calorie sweeteners are often already managing their weight, blood sugar, diabetes, or other cardiometabolic conditions. These underlying health factors are independently linked to cardiovascular risk and may help explain the association observed in the study.
